Maybe because Safari saves settings in plist-files which are not transactional and are prone to damage.

The log isn't useful indeed. In history for this nzb item - is the category set to TV Shows? To check the destination directory assigned to that item please open in a browser the url http://nzbget-ip:nzbget-port/jsonrpc/history. Please post it via pastebin, you can remove all other items to not expo...

Renaming is usually performed either via another automation app (Sonarr, Radarr) or using post-processing scripts, for example VideoSort.
