I have recently started having issues with renaming and file placement. I use Sonarr with nzbget and here's what I get:

And my and NZBGet Settings:
I'm getting similar results with Radarr and movies. What settings do I need to change: BTW, inside the folder there often exists an "Unpack" folder with the "raw" file name therein.
Anything else needed to analyze this?

NZBGet creates a directory with names same as nzb-name and unpacks all files there. It doesn't do any renaming at all. There are post-processing scripts which do renaming though.
In combination with Sonarr/Radarr these apps usually organize files downloaded by nzbget.
From your screenshots it looks like nzbget is doing its part properly.

_unpack is temporary folder created during processing. At the end the files are moved into parent directory and _unpack is deleted. If you have the _unpack that means the processing ended with errors. Check the log of each such item - in history click on an item, then on button "Log".
